Open an EverGreen Savings account

Step 1 is the bank's own application form (a mock — it goes nowhere). Step 2 is the live DPSuite consent form: it renders the published DPDP notice for Marketing communications and records your decision on the consent ledger, with a receipt hash.

This form is a stage prop — nothing you type here is submitted anywhere. Identity for the consent record is collected by the DPSuite form on the right, so the grant is identified and resolves to a real Data Principal.

Step 2 — Consent (rendered by DPSuite)

One application, two purposes. Nothing is pre-ticked (Section 6(1)): you must tick the mandatory consent yourself to apply, and the optional one is freely left off — the application proceeds without it.
